Double Eyelid Surgery
Create Naturally Beautiful, Brighter & More Defined Eyes
The eyes are one of the most expressive and noticeable features of the face. They influence how others perceive your emotions, personality, and overall appearance. Well-defined, balanced eyes can create a brighter, more youthful, and approachable look, while the absence of a natural eyelid crease or asymmetrical eyelids may make the eyes appear smaller, heavier, or less expressive.
Double Eyelid Surgery, also known as Asian Blepharoplasty, is one of the most popular cosmetic procedures in South Korea. It is designed to create or enhance the upper eyelid crease, making the eyes appear larger, more defined, and naturally attractive while maintaining harmony with your facial features.
At Sketch Plastic Surgery Korea, we believe that beautiful eyes are never created using a one-size-fits-all approach. Every double eyelid is carefully designed according to each patient’s facial anatomy, eye shape, skin thickness, lifestyle, and aesthetic preferences to achieve elegant, natural-looking results.
What is Double Eyelid Surgery?
Double Eyelid Surgery is a cosmetic procedure that creates a visible crease in the upper eyelid or improves an existing one.
Many people, particularly those of East Asian descent, naturally have monolids, meaning there is little or no visible upper eyelid crease. Others may have uneven eyelids, multiple folds, or eyelid creases that have changed over time due to aging.
Creating a well-designed eyelid crease can:
- Make the eyes appear larger and brighter
- Improve eye definition
- Enhance facial balance
- Improve eyelid symmetry
- Create a softer or more elegant appearance
- Make eye makeup easier to apply
- Restore a youthful appearance when aging has altered the eyelids
The goal is not simply to create a fold, but to design an eyelid that complements your unique facial features while preserving your natural beauty.
Why is Double Eyelid Surgery So Popular?
Double Eyelid Surgery is one of the most commonly performed cosmetic procedures in South Korea because it provides subtle yet transformative improvements with relatively minimal downtime.
Rather than dramatically changing the face, the procedure enhances the natural shape of the eyes by creating better definition, improving symmetry, and allowing more of the eyes to be visible.
Many patients choose Double Eyelid Surgery because they wish to:
- Create a natural eyelid crease
- Make the eyes appear brighter and more open
- Improve facial harmony
- Correct uneven eyelids
- Achieve a more youthful appearance
- Enhance makeup application
- Increase confidence in their appearance
At Sketch Plastic Surgery Korea, our philosophy is to enhance your natural features rather than create an artificial or exaggerated appearance.
Who is a Good Candidate?
Double Eyelid Surgery may be suitable if you:
- Have monolids and wish to create a natural crease
- Have uneven or asymmetrical eyelids
- Have multiple or poorly defined eyelid folds
- Want larger or brighter-looking eyes
- Desire a more refined eyelid shape
- Have realistic expectations
- Are in good overall health
During your consultation, our surgeons carefully evaluate your eye anatomy, skin thickness, muscle strength, and facial proportions to determine the most suitable surgical approach.
Types of Double Eyelid Surgery
At Sketch Plastic Surgery Korea, two primary techniques are available.
This technique creates the eyelid crease using specialized sutures placed through several tiny puncture points without making a full incision.
It is generally recommended for younger patients with thin skin, minimal excess tissue, and good skin elasticity.
Benefits
- No long incision
- Minimal scarring
- Faster recovery
- Less swelling
- Shorter procedure time
However, this method is not suitable for everyone, particularly patients with thick skin, excess fat, or significant skin laxity.
The incisional method creates the eyelid crease through a carefully placed incision along the natural eyelid fold.
This technique allows the surgeon to remove excess skin, muscle, or fat while creating a durable and well-defined crease.
It is often recommended for patients with:
- Thick eyelid skin
- Excess upper eyelid fat
- Aging eyelids
- Multiple folds
- Loose or sagging skin
Because the crease is created with direct tissue adjustment, the results are generally more stable and long-lasting.
Double Eyelid Crease Designs
Choosing the right crease design is just as important as choosing the surgical technique. At Sketch Plastic Surgery Korea, every crease is individually designed to complement your facial proportions rather than following a standard template.
The crease begins close to the inner corner of the eye and follows the natural eyelid contour.
This design creates a soft, elegant, and natural appearance and remains one of the most popular choices among Korean patients.
The crease begins close to the inner corner before gradually separating from the lash line.
This balanced design combines natural beauty with increased eye definition and is one of the most requested styles among international patients.
The crease sits slightly higher than an In-Out crease while maintaining a soft transition.
It creates brighter, more open-looking eyes without appearing overly dramatic.
The crease remains separated from the inner corner throughout its length, creating a larger and more defined appearance.
This design is often chosen by patients who prefer a bold, glamorous eye shape or naturally have facial features that complement a higher crease.
Our surgeons carefully recommend the crease design that best suits your anatomy, skin condition, and personal preferences.
Can Double Eyelid Surgery Be
Combined with Other Procedures?
Yes. Many patients achieve the best overall results by combining Double Eyelid Surgery with complementary eye procedures.
These may include:
- Ptosis Correction
- Epicanthoplasty
- Lateral Canthoplasty
- Upper Blepharoplasty
- Sub-Brow Lift
- Lower Blepharoplasty
- Under-Eye Fat Repositioning
- Facial Fat Grafting
Combining procedures allows the surgeon to improve both the function and aesthetics of the eyes while maintaining natural facial harmony.
The Sketch Plastic Surgery Approach
At Sketch Plastic Surgery Korea, we believe that every pair of eyes is unique.
Rather than creating identical eyelids for every patient, our surgeons carefully analyze:
- Eye shape
- Facial proportions
- Brow position
- Skin thickness
- Muscle function
- Existing eyelid crease
- Personal preferences
- Ethnic characteristics
Using this comprehensive evaluation, we develop a personalized surgical plan that enhances your natural beauty while preserving your individuality.
Our philosophy is simple:
Beautiful eyes should look naturally yours—not surgically created.
Surgery Time & Recovery
The surgical technique determines the length of the procedure and recovery period.
Most Double Eyelid procedures require approximately 30 to 90 minutes.
Depending on the procedure, surgery is typically performed under local anesthesia with intravenous sleep sedation, allowing patients to remain comfortable throughout the operation.
Most patients return home the same day after a short recovery period.
Initial swelling and bruising are expected during the first week and gradually improve over the following weeks. While most patients resume daily activities within 7–14 days, the final eyelid shape continues to refine over the next 2–3 months as healing progresses.

Frequently Asked Questions
The surgery does not enlarge the eyeball itself, but by creating a well-designed eyelid crease and improving eyelid opening when appropriate, the eyes usually appear brighter, larger, and more defined.
Both methods can provide long-lasting results. However, the incisional technique generally offers greater long-term stability, especially for patients with thicker skin or excess eyelid tissue.
Incisions, when used, are carefully placed within the natural eyelid crease, where they usually become well concealed as healing progresses.
Absolutely. Many male patients choose Double Eyelid Surgery to create a more refreshed and defined appearance while maintaining masculine facial characteristics.
Yes. Many patients benefit from combining the two procedures when weak eyelid muscles contribute to heavy or sleepy-looking eyes.

Double Eyelid Surgery, just like its name is a cosmetic surgery that makes a new double eyelid or changes the shape, symmetry or height of an existing double eyelid. It is also called Asian Blepharoplasty because this procedure is quite common and popular among people with Asian Ethnic backgrounds and there are relatively more people with Asian Ethnic background who have monolid eyes. In Asian beauty standards, having a decent Double Eyelid is preferred because that makes the eyes look bigger, more open, symmetric and more defined.
SURGICAL METHODS
There are two main types of the Double Eyelid Surgery based on the incision method used. Non-incisional Double Eyelid Surgery and Incisional Double Eyelid Surgery. Depending on the patient’s condition and desires, both surgeries aim to get the similar results which is a new or improved double eyelid. Not every patient is a good candidate for Non-Incisional Double Eyelid Surgery and not every patient is a good candidate for Incisional Double Eyelid Surgery.
Non-Incisional Double Eyelid Surgery
The type of double eyelid surgery in which few puncture points are made and with using dissolvable threads, a crease is made as per the desired design. There is no linear incision and no skin is removed. This is good for people with thin skin and are in younger ages. If the skin is thick or saggy, this is not the best method for which an in-person assessment is needed with the surgeon.
Double Eyelid Crease Types:
Our surgeons are experts in the following crease types for the double eyelid line surgery and the final design is made as per the patients desires.
In-Line Crease:
This is the typical soft and natural crease type popular among Koreans.
Out-Line Crease:
This is the type of crease that is more open and defined for a bold feature.
In-Out Crease:
The type of crease that is a combination of both in-line and out-line types.
Semi-Out Crease:
This is the type of crease that has a height a little lower than a typical out-line type.
Anesthesia
At Sketch Plastic Surgery, the Non-incisional Double Eyelid Surgery is done with Sleep Sedation along with some amount of local anesthesia. You will be in a deep sleep for around 30-60 minutes and once you wake up, you will be moved to the recovery room and kept there for as long as you take to completely recover from the anesthesia. Usually it takes around 45-60 minutes to feel completely fine and be discharged from the hospital. Our staff will put you in a taxi after explaining all post-surgery instructions and handing over the medication
Downtime
The downtime for non-incisional double eyelid surgery is short because there is no linear incision involved. Also there is no external stitching, so stitch removal is not needed. The stitches used are dissolvable. There will be some swelling and bruising which will peak on the 3rd and 4th day post surgery. Between 7-14 days, most of the swelling and bruising will be over, however some residual swelling and bruising will be there for 1-2 months. The final result will be visible in around 3 months.
Medication
After the surgery, the patient has to take medications for 5 days. The medications will include antibiotics to avoid any infection, anti-inflammatory, pain-killer, and stomach control medications. These medications must be taken 20 minutes after each meal. With the antibiotic medicines, the patient’s stomach could be upset, so avoiding spicy food for the first 5 days would be recommended by our surgeon. If any patient has an allergy with certain medicines, our surgeon will make adjustments accordingly.
Incisional Double Eyelid Surgery
This is the type of double eyelid surgery in which a linear incision is made on the upper lid and new or an improved double eyelid line is made. The height and design of the crease is set before the surgery and the incision is done accordingly. The design could be higher or lower double eyelid lines, in-line, in-out line, semi-out line or out-line shapes of the double eyelid lines. In this method, some saggy or droopy skin is removed and a deep eyelid line is made. This method is good for people with thicker skin and droopy and saggy skin mainly because of aging. It also solves multiple fold issues. In comparison to non-incisional double eyelid surgery, the incisional double eyelid surgery creates an internal adhesion along the crease line to create a more natural and a longer lasting outcome. Sometimes other procedures like Ptosis Correction, Epicanthoplasty, Lateral Canthoplasty etc are done together for better overall outcome.
